Overview
The Health Registrar records and issues marriage, birth and death certificates as well as executes voluntary paternities. A birth, death or marriage certificate may be requested by mail by submitting the appropriate form, photo ID of the applicant, and the $10 per copy fee (payable to the City Clerk) to:

A birth record will only be released to the individual (over the age of 18) on record or a parent listed on the record.
Death certificates will only be released to a spouse, sibling, parent or living child of the person named on the record.
Marriage certificate copies will only be released to the spouses named on the certificate, unless the requester has a NYS Court Order to obtain a copy.
An attorney can always request any record on file on behalf of an authorized individual.
